Please note that all Unity Health Care centers are closed on the first Thursday of each month from 2:00 p.m. to 5:00 p.m.
Important Information*
When you call Unity’s patient scheduling center, please identify yourself as a Providence patient and be prepared to identify your provider’s name and/or location you would like to be seen to assist in scheduling an appointment.
On or after June 8, your medical records from Ascension will be accessible at Unity Health Care, following a secure transfer in accordance with federal and local privacy laws, to support continuity of care.
The Providence AMG medical practices currently at 1140 Varnum St. – Suites 100 and 110 – will be vacated after the transition, and those providers and team members will move into one of the suites mentioned above (1160 Varnum St. NE Suites 312 and 317).
Introducing MyChart, Your New Patient Portal
MyChart is a secure, free patient portal app and website created by Epic, our Electronic Health Record system, that lets you manage your health information, message doctors, view test results, and schedule appointments on your phone or desktop. It also provides 24/7 access to your medical records across different providers. If you need help setting up MyChart, our patient scheduling center staff can assist you by phone or during your next visit. Please call 202-469-4699 for assistance.

7:45a.m. - 4:30p.m. or online anytime.

At Unity you have access to four pharmacy locations. You can go to your appointment and pick up your prescription all in one location!
