Starting Monday, December 7, all services at Columbia Road Health Center (1660 Columbia Road, NW) will be permanently transitioned to Upper Cardozo Health Center (3020 14th Street, NW).
Q: Why are services moving to a new location?
A: Services are moving to Upper Cardozo so we can offer our patients more. At Upper Cardozo, patients can receive primary care and a wide range of specialty services in a building that is wheelchair accessible and steps away from both the metro and bus routes.
Q: When is the last day I can receive services at Columbia Road?
A: The last day Columbia Road will be open is Thursday, December 3, 2026. Any appointments scheduled before or on December 3 will be held as scheduled. Appointments scheduled for any date after December 3 will be held at Upper Cardozo.
Q: Can I continue seeing the same provider?
A: Yes! All medical providers from Columbia Road will move to Upper Cardozo. You can expect to see the same team in your new health center location. If your usual provider is unavailable, we will help you schedule an appointment with another qualified member of our care team.
Q: What services are available at Upper Cardozo?
A: All of the services that were available at Columbia Road will also be available at Upper Cardozo. In addition to those services, you will also have access to an on-site pharmacy, dental care, WIC, Behavioral Health, Podiatry, and OBGYN, all in the same building where you see your primary care provider.
Q: Do I need to complete any paperwork before my first appointment at Upper Cardozo Health Center? Is there anything I need to bring?
A: You do not need to complete any new patient paperwork for your first appointment at Upper Cardozo because your record is already in our system. However, if your address, phone number, insurance, or other personal information has changed, please let the registration staff know so we can update your information in our system. Additionally, you should bring the following things to every appointment at any Unity Health Care location:
- A photo ID
- Your insurance card (if you have one)
- A list of your current medications
