Your One-Stop-Shop for Open Enrollment Resources and Information

The 2021-2022 Open Enrollment Window is OPEN! This is your annual opportunity to make changes to your benefits, beneficiaries, emergency contacts, and more. Enrollments must be completed online through the ADP Workforce Now Employee Self-Service portal between Monday, Nov. 1, and Sunday, Nov. 18. Use the resources below to learn more about employee benefits offered by Unity.

Many benefits will remain unchanged in the new year, but some adjustments have been made to medical plans, life insurance offerings, and disability, including several rate changes. Benefit summaries are available on ADP and will be added to the Additional Resources section of upcoming email communication. If you have any questions, email oe@unityhealthcare.org

ADD OR UPDATE INFORMATION FOR YOUR DEPENDENTS/BENEFICIARIES

Open Enrollment is a great opportunity to provide and update dependent Social Security Numbers (SSNs) if they are not currently on file with the organization, to maintain compliance with the Affordable Care Act (ACA). Note: social security numbers are required (if applicable) for dependents covered under medical, dental, and vision coverage.

Adding or Removing Dependents

Employees may add or remove eligible dependents from their benefit plans during Open Enrollment.  Additional documentation may be required before the dependent is approved.  Eligible dependents are:

  • Spouse – Marriage Certificate is required before approval;
  • Domestic Partner – Registered Domestic Partnership Certificate is required before approval;
  • Biological, Adopted or Court Appointment Guardianship of a Child – Birth Certificate, Certificate of Adoption or Court Guardianship Documents

Required Documentation may be uploaded directly on ADP Workforce NOW. Enrollment for these dependents will not be processed without all required documentation.

ENROLL OR MAKE CHANGES TO YOUR BENEFIT ELECTIONS

All benefit elections (including changes) are made through the ADP Workforce NOW portal.  ALL benefit elections/changes must be completed and submitted by 11:59PM on November 14th.  You will not be able to make changes after this date unless you have a Qualifying Life Change/Event (please reference end of this section for examples of QLEs).

  • Change in Marital Status
  • Death of a Family Member
  • Family Member Gains Coverage
  • Family Member Loses Coverage
