ROCC provides discharge planning and chronic disease maintenance services for the residents of the District of Columbia that have been recently released from federal prisons.

Collaboratively operating under the guidance of Mayor Anthony Williams’ Comprehensive Reentry Strategy for Adults in the District of Columbia and in partnership with United States Department of Justice’s Court Services and Offender Supervision Agency (CSOSA), the Federal Bureau of Prisons (FBOP), the District of Columbia Department of Health (DOH) and funded by the U.S. Department of Health and Human Service’s Bureau of Primary Health Care (BPHC), Unity offers services within area halfway houses and within the Unity’s First Street Health Center to assist residents with medical discharge planning and establishing a “medical home”
