COMMUNITY HEALTH PROMOTER PROGRAM
CHPs are outreach workers who assist hard-to-reach individuals, including pregnant teenagers, teen mothers, newborns, children and senior citizens. Based at Unity’s Congress Heights Health Center, these outreach workers help patients comply with their medical regimes and keep appointments with their providers, often accompanying the patients to the appointments. They also become part of the patient’s support system, helping them complete Medicaid applications, obtain food vouchers, get clothing and infant supplies, make travel arrangements, and so on. Each year, Unity’s Community Health Promoters make at least 1,400 home visits in Wards 7 and 8, which are the wards with the poorest health indicators and the largest hard-to-reach population. Through a cooperative agreement with Georgetown University Hospital, medical residents often accompany community health promoters when they visit patients, and these residents have been instrumental in researching avenues for patients to receive medications through drug company programs.
