How do I get an appointment?
Call the Teen Appointment Line at 202 469 4699 and press Option 2.  You will be asked for your address, phone number, date of birth, and your parent or guardian’s contact information.  We will not notify your parent/guardian that you are making an appointment, but we need their contact information in the event of an emergency.

What information does a teen need to bring to an appointment?
If you have insurance and have your insurance information, please bring it along with your photo ID.

If I don’t have insurance or don’t know my insurance information, can I still receive health care services at Unity?
Yes, you can. We can help you sign up for insurance and you will not be charged for your visit if you are a teenager without insurance.

My friend recently saw a Unity provider that s/he really liked. Can I make an appointment to talk to a provider about my questions even though I’m not a current patient?
Yes!  We are accepting new patients.
