What if I want to come in for a physical for school or sports team– does my parent or guardian need to be with me?
Yes, your parent or guardian needs to be with you for this appointment.  If they cannot come, they need to send a note with you saying that you can be seen by the health care provider that day.  They also need to be available to answer any questions by phone in case the health care provider needs to reach them.

My school sent home a letter saying I am missing some of my shots.  Can I get my shots at Unity?
Yes, please call the teen appointment line and make an appointment.  We offer all of the required vaccines for school and the optional ones, like HPV vaccine and flu vaccine.

I have questions about school or getting a job.  Is there someone at Unity who can help me with this?
Yes, our clinics have case managers that can meet with you and help you with questions about school or GED, employment, summer jobs, and help you plan for your future.
