Eligibility / Health Fees

All students attending Eastern High School are eligible. However, in order to receive health services all students must have a signed parental consent form.

Students 18 years of age or older are not required to have a signed parental consent.

For students who are uninsured, there is no charge for health services.

For students with insurance through private companies, state or federal government programs, fees for services may be billed to the insurer.
